Please fill out the form below as completely and accurately as possible. We ask that you do this because many of our rescue dogs haven’t had the best lives so far, and we want to make sure that they go to foster homes that are best suited to their unique personalities.

  8. Are all of your pets spayed / neutered?
  9. Are you willing to provide food for the foster dog?
  10. Do you have a completely secure, fenced yard?
  11. Do you own or rent?
  12. Rescue dogs have often been through a lot of stress and have not had a stable environment. We emphasize routine and training in a positive manner without the unnecessary use of harsh leash corrections, choke collars, rolling, pinning and hitting. Do you accept this?
  13. Do you feel that you can provide a routine and the necessary basic training in a positive manner (this may include but is not limited to: teaching manners, basic obedience, solving a problem behavior, instilling confidence in shy dogs, calming nervous and fearful dogs)?
  14. Would you like more resources for training and behavior?
  15. Are you able to take your foster dog to an obedience class or private session?
  16. Are you able to take your foster dog to a veterinarian for vaccinations or other medical care
  17. Are you willing to act as primary contact for the dog?
  18. If for any reason you are unable to keep the foster dog or are unhappy with him/her, you may notify New Hope Dog Rescue (NHDR) and make arrangements for the dog to be placed into another home. Please be prepared to give your foster home time to settle in, do not expect them to want to play or necessarily interact with your other pets or even you to begin with. Some dogs are very scared when they move homes and need some time and personal space, let them show you when they are ready. By submitting this form you have read and understand the above information and agree that you will not hold NHDR coordinators or any representatives associated with NHDR responsible for any destructive or aggressive behavior exhibited by the rescue dog, or for any other problems that may arise pursuant to its foster care.
